Implications of community integrated health and social care system in Japan

Abstract
In 2018, the Ministry of Health and Welfare announced a variety of plans for the Korean "Integrated community care program". Japan has experienced aging before Korea, so it is subject to various banch-marking. The low birth rate and declining population are common concerns between Korea and Japan. The purpose of this study is to review the current status of community care in Japan and draw lessons and implications for the integrated community care policy of Korea's aged society in the future. The Japanese community integration care system began in the 1970s with "community integrated health and social care" at Hiroshima Prefecture Public Mitsugi General Hospital. It was only after 2000 that this became universal and developed into a national policy. I would like to make some suggestions in Japan's review of community-integrated care system. First, Japan's community-integrated care system came together with the reform of the overall medical system. The whole medical system is being carried out in the midst of a shift from hospital admission to community care. Second, the Japanese municipal government, cities, towns and villages, plays an important role as the main body of the establishment and implementation of community integration care. Municipal government is an insurer of long-term care insurance and serves as an insurer of local medical insurance, and cooperates with private medical institutions to coordinate the care of the entire local residents. Third, activities are carried out to coordinate and support the medical care, social care, daily life support, and various areas of the communities in an integrated way. There are various services such as discharge support programs, home care, visiting nursing, visiting care, day care, short-term hospitalization, as well as new organizations and personnel such as community integrated care centers and care coordinators. As a conclusion, it is important to understand each country's operational mechanism rather than simply comparing the status quo of Japan and Korea. It will be important to figure out how Japan's community integration care changes amid a longer-term trend.
